Lost / Stolen Passport

Replace a Lost or Stolen U.S. Passport

If your U.S. passport book or card has been lost or stolen, you generally need to apply in person for a replacement using Form DS-11. US Passport Assist helps you prepare the application and supporting package before your acceptance-facility visit.

The form: DS-11 + DS-64 (in person)

Once the DS-64 is filed the old passport is permanently invalid, even if found later. If it was stolen: a police report is not required by the State Department, but we recommend filing one — especially if other documents were taken with it.

Government fees

$130 government application fee, paid to the U.S. Department of State, plus a separate $35 acceptance fee paid directly to the acceptance facility (accepted payment methods vary by location). Optional expedited service adds the State Department's $60 expedite fee plus $26.87 for 1–3 day return delivery of a passport book ($86.87 together).

What Should You Know Before Applying?

Reporting your valid passport as lost or stolen helps prevent unauthorized use and is an important step in the replacement process. US Passport Assist guides you through the reporting requirements and helps prepare the forms needed for your replacement application.

Reporting your passport does not replace it. Once reported and canceled, it cannot be used for travel, even if you find it later.

If your missing passport has already expired, you do not need to report it lost or stolen. You will still need to apply for a new passport if you need a replacement.

What Do You Need to Replace a Lost or Stolen Passport?

You will generally need:

  • Form DS-11 for your replacement passport application
  • Proof of U.S. citizenship
  • A government-issued photo ID
  • One printed passport photo that meets government requirements
  • Details about when and where your passport was lost or stolen
  • Form DS-64 (Statement Regarding a Lost or Stolen U.S. Passport), when required
  • A copy of the police report, if you filed one
  • Applicable government fees

US Passport Assist will explain which forms, documents, and copies are required and help you prepare your replacement application package. Applicants under age 18 must also meet age-specific parental consent or awareness requirements.

How Does the Application Process Work?

  1. Provide Your Information

Complete the online application and provide details about your missing passport, including whether you have already reported it lost or stolen.

  2. We Prepare and Review Your Application

US Passport Assist prepares Form DS-11 and any additional required forms, reviews your information and passport photo, and provides a personalized document checklist.

  3. Receive Your Application Package

We mail the prepared application package to you with the required forms, personalized checklist, and clear instructions for your acceptance-facility visit.

  4. Visit a Passport Acceptance Facility

Bring your prepared package and required supporting documents to an authorized passport acceptance facility. Do not sign Form DS-11 until instructed by the acceptance agent.

  5. Submit and Track Your Application

Pay the applicable government fees and submit your application through the acceptance facility. After submission, you can follow the progress of your passport application.

US Passport Assist helps you prepare your replacement application, organize the required documents, and understand each step before your in-person visit.

Common questions

What do I do if I lost my passport?

Report the loss with Form DS-64 and apply for a replacement in person with Form DS-11 — both can be submitted together at an acceptance facility. Reporting invalidates the lost passport immediately, which protects you from identity misuse.

Do I need a police report for a stolen passport?

The State Department does not require a police report to replace a stolen passport — Form DS-64 is the required theft report. We still recommend filing one, especially if other documents or cards were taken with it.

What if I find my passport after reporting it lost?

It stays invalid. Once a DS-64 is processed the passport cannot be used for travel and should be mailed to the State Department if found.
