Passport Renewal
- Your most recent passport is undamaged and in your possession
- It was issued when you were 16 or older
- It was issued within the last 15 years
DS-11
New Adult Passport
- You’ve never had a U.S. passport
- Your previous passport was issued before you turned 16
- Your previous passport was issued more than 15 years ago

Child Passport
- The applicant is under 16 (child passports are valid 5 years)
- Child passports cannot be renewed. A new passport application must be submitted.

Lost / Stolen Passport
- Your passport is lost and you need a replacement
- Your passport was stolen and you need a replacement

Damaged Passport
- Your passport is valid, but damaged, and was issued when you were at least 16 years old

Name Change
- You legally changed your name through marriage, divorce or a court order
- Your current or expired passport shows your previous name

Second Passport Book
- Your previous passport was issued within the past 15 years, and you were at least 16 years old
- You currently have your passport in your possession but need a second passport book

Passport Card
- Wallet-sized U.S. passport that proves citizenship and identity
- Valid for land and sea travel to Canada, Mexico, Bermuda, and certain Caribbean destinations
- Apply for a new passport card or renew an eligible existing card
- Get the card by itself or together with a passport book
